Constance Way can consider referrals from a variety of agencies (principally Social Services or Health Authorities) anywhere within the United Kingdom. Assessments are made in collaboration and partnership with the referring agency.
This is an attractive detached property, with a light and airy conservatory, in a suburb of Leeds. The project comprises six self-contained studio flats, each with a separate bathroom and kitchen, together with communal living area and laundry room. Constance Way is accessible to those with limited mobility, including wheelchair users.

The Constance Way support team consists of both Deaf and hearing staff members who have been selected for their personal qualities, skills and experience and who are familiar with and respect the culture language and community of Deaf people.
The staff team comprises the Manager, Team Leader, Support Co-ordinator, six Support Workers and a part-time administrator. All staff use BSL as the main language for the project. The building is fully equipped with environmental aids such as text telephones, visual fire alarm and doorbell system.

Constance Way operates on the ethos of independent community living. Service provision and the level of support required is determined, with full involvement of the tenant, following a careful assessment of individual need. Prior to admission, individual packages of care are determined and developed through the contribution and collaboration of all relevant agencies.
SignHealth provides emotional and structured support in line with the agreed aftercare plan which is reflected in the development of a comprehensive package of support. Tenants are encouraged to take a pro-active part in the running of the project and every aspect of daily living. SignHealth promotes and respects tenants’ independence, dignity, rights, choices and decision making through partnership, advocacy and empowerment.

Constance Way assists service users to achieve an independent and balanced life by working closely with families, social workers, medical professionals, specialist agencies, voluntary groups and colleges.
The house is well situated in a mixed residential/business area of Leeds close to the universities and within walking distance of shops and bus routes, key facilities.
Following the completion of assessments, approval of referral and the securing of funding, the new tenant is encouraged to be fully involved in every aspect of their transition to their new home. A key worker is allocated and the development of a structured package of support is initiated.
Constance Way works extensively and intrinsically with specialist mental health practitioners and through regular multi-disciplinary reviews and case conferences, SignHealth are able to secure and develop the quality of provision and support for our service user group.[clear-line]
