Polestar can consider referrals from a variety of agencies (principally Social Services or Health) anywhere in the United Kingdom. Referral enquiries should be directed to the Project Manager. Assessments are made in collaboration and partnership with the referring agency.
Polestar is a purpose-built registered care home in Sutton Coldfield, run on the supported living model. It is in a quiet residential area, and has six two-bedroom flats
Each of the two-bedroom flats has its own bathroom, open-plan kitchen and living/dining room. This encourages residents to pursue an independent lifestyle, giving them the chance to host family and friends, and to develop their social skills. There is also a communal area, which includes a training kitchen and a garden.
The Polestar staff offer support to tenants 24 hours a day. The team is made up of a Housing Manager, Team Leader, Administrator and Housing Support Workers. It includes both Deaf and hearing staff who all use British Sign Language as the main language of the home.
The staff bring a variety of cultural and social care values. Importantly our Deaf staff provide positive role models for the tenants.
what we do
Polestar operates on the basis of person-centered planning, with the tenants as lead decision makers. Each tenant works with their key worker from the support team on identifying and achieving their targets, and support plans are reviewed regularly.
In the monthly house meetings tenants and the support team formally discuss ideas and negotiate choices to do with the running of the home.
Who we work with
Polestar works in partnership with Trident Housing Association, and the Jasmine Suite (National Deaf Mental Health Services), as well as other professional and cultural teams within the Midlands area.
The home assists tenants to achieve independent and balanced lifestyles, by working closely with families, social workers, medical professionals, specialist agencies, colleges, voluntary groups and the community.
The staff team will support tenants to gain and maintain a fulfilling lifestyle, self-esteem and independence. They will achieve new skills and become actively involved in the local and broader communities.
New tenants are encouraged to be fully involved in all aspects of their transition to their new home. A key worker is allocated and a structured package of supports developed, based on individual need. That package is reviewed after one, three and six months.
